It was okay. The twist did take me by surprise, but

I personally dislike when mental illness is passed off as horror, especially when the main character is innocent.

I'm a bit torn on this one. I didn't love the writing, but I'm glad I stuck with it because the storyline ended up being better than I expected. The characters really got under my skin, which made me want to DNF, but now that I've finished reading/listening, I realize that was probably the point. So maybe the writing was actually a lot better than I was giving credit for.
