This main character is a piece of work (derogatory) but the story’s major elements are so relevant all these decades after it’s first publication. I don’t know yet how to reconcile that I hate the main character so much and yet feel that this should be required reading.
Full thoughts to come in an edit, though.
Graphic: Adult/minor relationship, Animal death, Body shaming, Bullying, Confinement, Cursing, Death, Emotional abuse, Infertility, Infidelity, Mental illness, Miscarriage, Misogyny, Rape, Self harm, Sexism, Sexual assault, Sexual content, Sexual violence, Slavery, Suicidal thoughts, Suicide, Torture, Toxic relationship, Transphobia, Violence, Blood, Excrement, Vomit, Kidnapping, Grief, Religious bigotry, Abortion, Suicide attempt, Murder, Pregnancy, Gaslighting, Sexual harassment, War, Injury/Injury detail, and Classism
This book is explicit in every topic it addresses, and it is addressed through the mind of the main character who sexualizes everything, no matter how unnecessarily. It is often without warning, too, so you can’t even skip over it if you think it’s coming up. It’s also graphic in every other way, too.lexib13's review against another edition

I think a lot of people, especially coming from the TV show, want a rebellion narrative from this book. They want an action hero. And you don't get that. You get a flawed, thoughtful, broken woman, who just exists in a horrible world. A lot is implied or mentioned or considered, but this is rarely action-packed. Whole chapters are pondering, recollecting, processing. Living through dystopia is not exciting most of the time.
And so many people dislike the main character--I would say we don't need to like her. We just have to listen.

Moderate: Drug use, Infidelity, Suicide, Medical content, Grief, and Alcohol
Minor: Animal death, Homophobia, Infertility, Miscarriage, Torture, Excrement, Abortion, and War
The worst of it in my opinion is implied/alluded to, or told through moments/observations rather than actions. Nowhere near as traumatic as the TV show, but uncomfortable throughout. I tried to describe the things you need to know below. I'm sure I'm forgetting things--please be cautious. Sexual Violence:reddeddy's review against another edition

it's definitely white feminism, with racism in this religious-extremist totalitarian regime only mentioned once, in passing, at the very end. compared to another modern classic published just two years later, Beloved, it's kind of embarrassing.
cover design review: ★★★★☆. instant one-star deduction for having author's name larger than the title (sorry Margaret Atwood), and this is especially egregious; the title is relatively tiny. we all know minimalism has become excessive but i really like this one. the centering of the title is very satisfying, with approximately equal overhang of "HANDMAID'S" on either side of the dress. and the best part, get ready for your mind to be blown: see that dot in front of the Handmaid's head? it's the eye of a face in profile looking to the left hidden in the negative space. genius
