I don’t know how the anime ends, but apparently this ending is more satisfying? I’ve heard some things, so I’m definitely going to have to finish the anime for comparison. But given just this, I’m pretty satisfied.
It’s a bit odd, and apocalyptic for a bit there, otherworldly. But I’m okay with that, how the dystopian world and disease gets resolved.
I think that Shion is in a pretty good place at the end of the volume. The ending with him and Rat is sad, but I think that the very end has enough hope, that I can let myself believe there’s a happy ending there. And I like the similarities to the beginning of the series.
Overall, I’m pretty satisfied. It left me pretty satisfied, at least.

I read the first 5 volumes of this series last year, and, when I finally got the last volume, I re-read the whole thing. I did not enjoy it nearly as much the second time. The overall story itself was okay (except the end; see next paragraph) but I think the execution tripped it up. Also, it could have been longer and delved deeper into the story. Actually, now that I'm thinking about it, it would have been great as a dark seinen fantasy series (minus the shounen-ai elements, obviously).
On to my opinion on the ending. While reading this series, I got the feeling that there was an epic conspiracy by the leaders of No. 6 to use the general population for...something (whether that was just to control them absolutely or to use them for science experiments or something else). But in the end, it just felt small. Too easily resolved. And Shion bounced back (and forward and up and down) WAY too easily and quickly to be realistic.

Spoiler
The main bad guy (name?) was killed (again, a little too easily) and the correctional facility was destroyed, but what about all the people that apparently knew what was going on and just let it or helped with it (after all, the main bad guy didn't really do any of the grunt work or administration, he was just the evil, crazy scientist in charge)?averypleasantpineapple's review

It's good that the ending is nearer to the novels than the anime version, but I'm still not a huge fan of it. Might have something to do with the gigantic wasp.

A more cohesive ending to the story than the anime managed. Very good series!
