This is definitely a crazy, off-the-rails horror book - it's not attempting to be realistic at all. I also don't think the characters are meant to be fleshed out because the book itself is so short, and it's also not super well-written, so if you're looking for good writing, this is not it.
However, I still really enjoyed it! It was super twisty, and I think all of the twists took me by surprise because they were so crazy. I loved how interconnected the patient files were and how each of the endings surprised the hell out of me! This author is not particularly skilled at characterization or writing, but they sure as hell know how to write twists.
The ending was also pretty off-the-rails and I liked how everything came together, although I did feel that all the connections were perhaps a bit too convenient. The very end was a bit of a cliffhanger in terms of what happens to Dr Harper, which really makes me want to pick up the sequel, but I'll probably wait a bit before I do because the amount of off-the-rails plot packed into this book was enough for me for a while.
