The premise of this book is quite interesting, and the way it lays out your expectations is the reason for the rating of the book itself.  When I just started reading the book gave me feeling like it was written for a movie but this feeling passed as I kept reading. This actually, makes sense as the writer of the book works as a screenwriter - which is why sometimes I felt like I was reading the script for future movie, rather than the novel itself.
I enjoyed the book a lot, since I love and adore books with the dark atmosphere with tired from routine doctor as it's narrator. I liked everything: the dark humor, the setting, gray morality if the narrator (as we look quite a lot in his addiction). BUT, the story  as a murder mystery evolved in the last 100 pages or so. Therefore, in this sense my expectations were not met. Maybe if the book were not advertised to me as a murder mystery, but as a life story of s doctor with a detective plot line, the rating would be higher, because as I said I enjoyed other parts of the book very much and had quite a good time reading it.
I would recommend this book to the lovers of medicine, as it shows the face of the Healthcare system with some brutality and honesty.
