Still Alice by Lisa Genova

Alzheimer's disease is bad enough when you're old, but to be diagnosed with early-onset is a terrifying thought, and we follow the process through Alice's eyes. Each chapter begins with a sort of recital of core facts, and through them - and Alice herself - we see the gradual disintegration of her mind as she becomes lost, and the heartbreak it sometimes causes.

Later reread: I don't know whether it's the Kindle or the new version, but the chapter leads were not present in the 10th anniversary copy.
